What Does MediShield Life Actually Cover?
Every Singapore citizen and PR has MediShield Life. But the average claim is just $1,541, while many hospital bills run into tens of thousands. Here is what it covers, what it does not, and where the gaps are.
What MediShield Life covers
MediShield Life is a basic health insurance plan run by the government. It is compulsory for all citizens and PRs. It covers:
- Hospitalisation at public hospitals (B2 and C wards)
- Day surgery at public and approved private facilities
- Outpatient treatments for specific conditions: chemotherapy, radiotherapy, renal dialysis, and immunosuppressants after organ transplants
Important: MediShield Life covers the bill, not the full bill. After the claim limit is applied, there are still two layers of cost:
- Deductible — the first $1,500–$3,000 per year that you pay yourself
- Co-insurance — 3–10% of the remaining bill above the deductible
You can use Medisave to pay the deductible and co-insurance portions.
What it does NOT cover
- Private ward costs above B1 limits
- Outpatient GP visits and routine check-ups
- Dental treatment
- Pre-existing conditions in the first year (for new PRs)
- Cosmetic and elective procedures
- Alternative medicine (TCM, chiropractic, etc.)
- Experimental treatments and unapproved drugs
The key point: even in a subsidised ward, MediShield Life does not pay 100% of your bill. You will always have some out-of-pocket cost from the deductible and co-insurance.
The coverage gap
These conditions have Ward C costs that far exceed the average MediShield Life claim of $1,541. The gap column shows how much the high-end bill exceeds the average claim.
| Condition | Ward C High | Gap vs Avg Claim | Multiple |
|---|---|---|---|
| Premature Birth | $105,916 | +$104,375 | 69x |
| Hernia | $60,973 | +$59,432 | 40x |
| Heart Valve Replacement | $46,215 | +$44,674 | 30x |
| Leukaemia or Lymphoma | $30,446 | +$28,905 | 20x |
| Bone or Joint Problems | $28,848 | +$27,307 | 19x |
| Kidney Failure | $27,665 | +$26,124 | 18x |
| Brain Surgery | $26,995 | +$25,454 | 18x |
| Back Surgery | $26,554 | +$25,013 | 17x |
| Poor Circulation | $24,820 | +$23,279 | 16x |
| Head Injury | $24,561 | +$23,020 | 16x |
MediShield Life helps, but it is not enough for serious conditions. A heart valve replacement can cost $105,916 in a subsidised ward alone, which is 69x the average claim.
What about Private wards?
MediShield Life claim limits are set for B2/C ward bills. If you choose a Private ward, the gap becomes enormous. These 34 conditions can exceed $50,000 in a Private ward:
Integrated Shield Plans (ISP)
An Integrated Shield Plan tops up MediShield Life. About 68% of Singaporeans have one. ISPs are sold by private insurers (AIA, Great Eastern, Prudential, NTUC Income, Singlife, Raffles Health Insurance) and offer coverage for:
- B1 ward or Private ward stays
- Higher claim limits for surgical and hospital bills
- Coverage at private hospitals
ISPs come with their own premiums, deductibles, and co-insurance. Riders can further reduce your out-of-pocket costs but add to the premium.
This is NOT financial advice. Consult a qualified financial adviser before making any insurance decisions. MediShield Life benefits and claim limits are subject to change by the government.
Key takeaways
- MediShield Life is a safety net, not full coverage. It covers subsidised ward bills with deductibles and co-insurance. You will always pay something out-of-pocket.
- The average claim is just $1,541. Over 1 million claims were made in 2024, but the average payout is modest.
- Serious conditions cost 10–60x that amount. 107 of 109 conditions have Ward C bills exceeding the average claim. Complex surgeries can cost $105,916 or more.
- ISPs bridge the gap for higher ward classes. If you want B1 or Private ward coverage, you need an Integrated Shield Plan on top of MediShield Life.
Frequently asked questions
Yes. MediShield Life covers cancer-related hospitalisation and approved outpatient treatments like chemotherapy, radiotherapy, and dialysis at subsidised wards. However, it does not cover the full bill. There are deductibles ($1,500–$3,000 per year) and co-insurance (3–10% of the remaining bill). For cancer treatment at Private wards, MediShield Life pays very little of the total cost.
MediShield Life covers treatment at private hospitals, but only up to the claim limits set for B2/C ward equivalents. Private hospital bills are typically 3–10x higher than subsidised ward bills, so MediShield Life covers only a small fraction of the total private hospital cost. You would need an Integrated Shield Plan (ISP) to get meaningful coverage at private hospitals.
MediShield Life is the basic, compulsory national health insurance that all Singapore citizens and PRs have. It covers subsidised ward (B2/C) bills with deductibles and co-insurance. An Integrated Shield Plan is a private upgrade that tops up MediShield Life to cover B1 and Private ward bills. About 68% of Singaporeans have an ISP. They are sold by private insurers and have additional premiums.
Yes. Medisave is separate from MediShield Life. You can use Medisave to pay for the deductible and co-insurance portions of your hospital bill that MediShield Life does not cover. There are annual withdrawal limits. The average Medisave balance is about $31,000.
MediShield Life does not cover: outpatient GP visits, dental treatment, cosmetic surgery, alternative medicine (TCM, chiropractic), pre-existing conditions in the first year for new PRs, congenital conditions diagnosed before age 30, and experimental treatments. It also does not cover the full bill even in subsidised wards because of deductibles and co-insurance.
